How to Get Started
EASY 3-STEP PROCESS
Give us a call, send an email, or fill out our contact form with your personal info and any care-related questions. We’ll then set up a Personalized Consultation just for you.
We’ll discuss your needs and concerns, involving your team. From there, we will assist in designing a customized care plan that works for you.
Once your care plan is finalized and our staff is trained to support your needs, we’re ready to begin services and support you on your journey!

Eligibility & Application
Anyone with a developmental disability diagnosed before age 22 who meets the required level of care.
Apply online via the BDDS Gateway or at your local BDDS office.
No—if you’re under 18, only your income and assets are counted (not your parents’).
Family Supports Waiver (FSW): Individualized supports
Community Integration & Habilitation (CIH) Waiver: Comprehensive services for higher needs
